Cost & Insurance

• $50 cash price when you pay at the visit
• State Medicaid plans (Medicaid, Amerigroup, CareSource, PeachState) must use the child’s primary doctor for physicals.

What to Bring

• School or league physical form, with the patient’s name and date of birth on each page and the parent/patient section filled out with each “yes” answer explained.
• Parent photo ID
